2019-2020 Transcript Request Procedures for Current Students

*Five days are required to process your request*

Transcripts for Colleges and Scholarships:

  1. All transcripts sent via Naviance are FREE.
  2. The first THREE printed transcripts are FREE, each additional is $3 per college/scholarship.
  3. There is no charge for your final transcript to be sent in June.
  4. Complete the online Transcript Request Google form to inform Ms. Oden of your request

Steps for Sending Transcripts via Naviance:

  1. Log into Naviance and add colleges/universities to your Naviance account.
  2. Link Common Application to your Naviance account (if necessary).
  3. Click “Request Transcripts”, select “Initial”, and choose the schools you need transcripts for.
  4. Note whether or not your college accepts electronic transcripts

Please note option D. These college/universities do not accept transcripts electronically. If this is the case, Ms. Oden will provide you with a free sealed copy of your transcript.

2019-2020 Transcript Request Procedures for Recent Graduates

  1. Complete the online Transcript Request Google form:
  2. Ms. Oden will send you an email confirming your request. Once you receive confirmation you must pay for the transcript using the Online School Payment link on the Berry Academy website.
  3. Once payment has been received, you will be notified when  your transcript will be available for pick-up or will be mailed per your request.

If you graduated prior to 2014, or if you need records other than a transcript, you must contact Student Records at 980-343-5424 or utilize the online request:


CMS offers an online secure and convenient way to request and receive student records. There is a $10 fee for each requested document plus $4 convenience fee. 

Documents that may be requested include:

  1. Certified transcript
  2. Documentation in order to obtain ID, passport, birth date or tax verification
  3. Immunization records for students who graduated after 1982

CMS Online Student Records Request Form:

Upon submission of your request, CMS Student Records department will retrieve and mail the record(s) to the address(es) designated on your form. Once mailed, your payment will be processed and you will receive an email confirmation and receipt. In the interest of security, and expedited service, phone and fax requests will no longer be accepted. 

Request for a student's complete cumulative record or for documents other than transcripts, immunization or ID verification, there is a $35 processing fee per record. Please allow up to four (4) weeks for processing.
